Baptistcare is one of WA's largest not-for-profit providers of residential care, at home services, retirement living and disability services, supporting individuals and communities in metro and regional areas for over 40 years, and employs over 1,500 staff.Baptistcare's At Home Services is using a fresh approach - working in partnership with customers, their carer’s families, medical practitioners and other stakeholders to find the best outcome for customers.  Our aim is to assist people to live fulfilling and independent lives within their community. The Clinical Service Facilitator will utilise their skills and knowledge as a Registered/ Enrolled Nurse, to provide high quality, flexible and personalised in-home care and support to customers.  The role will address clinical care needs that maintain the customer’s independent lifestyle whilst planning for their future needs. The Clinical Service Facilitator will be skilled in assessing and developing individual goals of care with consumers.
